Cologne, 07.06.2021. The Cologne-based digital health pioneer m.Doc has gained another partner for discharge management in hospitals: The start-up company Pflegeplatzmanager from Greiz in Thuringia has been part of the m.Doc Smart Health Platform since May 2021. Pflegeplatzmanager is a web-based communication platform for networking various players to be able to ensure controlled, cross-sector discharge and transfer management of patients in need of care.
Founded in 2018, 456 hospitals and rehabilitation clinics across Germany are already using the multiple award-winning service of Pflegeplatzmanager, enabling them to contact more than 28,000 follow-up care providers, for example from the areas of care and rehabilitation transition, hospital transfer, patient transport, and aid procurement.
Pflegeplatzmanager Managing Director Alexander Bauch welcomes the collaboration: “The integration into m.Doc’s offering takes us a big step further in our efforts to create overarching and supra-regional healthcare regions. All players in this complex ecosystem, consisting of clinics, post-acute care providers, payers and service providers, need to network ever more closely with each other. Clinics can now view free care capacities in real time directly in the m.Doc Smart Clinic patient portal, contact potential follow-up care providers, and transmit all relevant case data to them in an uncomplicated and data-protection-compliant manner. This relieves patients and relatives in situations that are often psychologically demanding, while at the same time significantly reducing research and documentation work for the staff.
