Discharge management service: m.Doc and Stiftung Gesundheit jointly develop new module for m.Doc Smart Clinic

From now on, m.Doc can access the comprehensive database of around 580,000 service providers from various medical fields of Stiftung Gesundheit. The new, jointly developed module based on this data takes discharge management to a new level, and not just for the medical and nursing staff in the clinics. For the first time, patients and their relatives are also actively involved in communication – in line with the m.Doc philosophy of patient-centered healthcare.

Cologne, Germany, March 23, 2022. m.Doc, the digital healthcare pioneer, and Stiftung Gesundheit are now pooling their expertise for greater transparency and optimal follow-up care after inpatient treatments. To this end, both partners have developed a new module for the m.Doc patient portal Smart Clinic. Here, Stiftung Gesundheit is contributing the knowledge and information from the structural directory of medical care, which it has maintained and updated for more than 25 years. It currently includes around 580,000 service providers who are actively involved in patient care. In addition to physicians in private practice, these include rehabilitation clinics and numerous non-physician health care professionals.

The new module integrates the information from the structure directory into the m.Doc Smart Clinic. As soon as a clinic determines the need for follow-up treatment within the scope of discharge management, all possible service providers can be contacted – selectable according to more than 4,500 characteristics such as specialty, therapy focus, contract types or region. The highlight is that patients and their relatives are directly involved in this communication via the patient portal, are informed at all times and can even help decide, for example, which doctor should take over the follow-up care. As a communication channel, the portal uses the usual tools within the telematics infrastructure – KIM and TI Messenger.
